A young male recently diagnosed with dengue fever and thrombocytopenia (low platelet count) presented with severe headache. He was initially treated at another center. Due to a deteriorating Glasgow Coma Scale (GCS), a CT scan of the brain was performed, which revealed an acute subdural hematoma with midline shift, despite no history of head injury.

Note: Dengue hemorrhagic fever with severe thrombocytopenia can lead to spontaneous intracranial hemorrhage such as subdural hematoma, even in the absence of head trauma. Severe headache in a patient with dengue should never be ignored—it may be a sign of a life-threatening complication and should be evaluated promptly with neuroimaging.

On a repeat CT scan, re-bleeding was noted. He subsequently underwent re-exploration and re-evacuation of the hematoma. After multiple platelet transfusions, his platelet count improved to 135,000/µL. His post-operative CT scan showed a satisfactory result.

Spontaneous Acute Subdural Hematoma - MRI

At the time of admission, his platelet count was 70,000/µL, and due to the high risk of re-bleeding, an evacuation of the subdural hematoma was performed.

His GCS gradually improved, and he progressed to a stable neurological status. He is now walking independently and is being discharged in a stable condition.
